About Us
East Toronto Health Partners (ETHP) is the Ontario Health Team (OHT) serving East Toronto. We are a group of more than 100 community, primary care, home care, hospital and social services organizations in East Toronto, as well as patient, caregiver and community advisors, working together to better connect local care and services.
We work together as an integrated team to make it easier for you to navigate the healthcare system and transition between healthcare providers and places where you receive care, making sure that you don’t feel lost or unsupported.

We are responsible for providing care and support to the 300,000 individuals who live in East Toronto communities, as well as an additional 75,000 patients who get their care here. Learn more about who we serve.
Our patient, caregiver and community advisors, as well as community health ambassadors, are important members of ETHP. They provide valuable feedback that help us co-design a more inclusive system that delivers high-quality care and equitable health outcomes for everyone.
What is an Ontario Health Team?
Ontario Health Teams, also known as OHTs, were introduced by the Ontario Government in 2019. They are a better way to organize and deliver services in local communities to provide patients with better connected, more integrated care.
Under OHTs, the healthcare providers who care for you (including hospitals, doctors and home and community care providers) work together as one coordinated team – no matter where they provide care.
There are more than 50 OHTs. Each one serves a specific geographic area. Learn more about Ontario Health Teams.
In September 2023, the Ontario Ministry of Health and Ontario Health (OH) named ETHP as one of 12 OHTs they will “accelerate” to deliver better care. This means, over the next few years, ETHP will receive support to rapidly implement improvements to patient care, and governance and operational structures.
