East Toronto Health Partners (ETHP) is the Ontario Health Team (OHT) serving East Toronto.
We are a group of more than 100 health, social and community service providers and organizations – as well as community members – who work together to improve the way people find and get care in East Toronto.
By working together as one coordinated team, rather than separately, we aim to make care – and the process of finding and getting care – more connected and easier to navigate for everyone. This approach improves access to services, reduces delays and duplication in care, and addresses gaps in service delivery.
We are responsible for providing care and support to the 300,000 individuals who live in East Toronto communities, as well as an additional 75,000 clients who get their care here.
Clients, families and caregivers, as well as community health ambassadors, are also important members of ETHP. They provide valuable feedback that help us develop services, programs and initiatives that serve our communities as effectively as possible.
What is an Ontario Health Team?
Ontario Health Teams, also known as OHTs, are a model of care that the Ontario Government introduced in 2019. They are groups of health, social and community service providers and organizations – as well as community members – who work together to improve the way people find and get care in their communities.
Examples of providers and organizations within an OHT include family doctors, hospitals, home care organizations, community agencies, mental health and substance use services, long-term care homes and more.
