Our cQIP is built on established frameworks and approaches for improving health for all, our OHT priorities and supporting COVID-19 recovery.
We’re proud to share that East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto, has developed a Collaborative Quality Improvement Plan (cQIP) for 2022/23. It aims to identify barriers and improvement opportunities related to cancer screening, alternate level of care (ALC) and mental health and substance use in East Toronto.
cQIPs are an important part of how OHTs, including ETHP, deliver higher quality, more integrated care to the communities they serve. Developed over the past eight months with patients, caregivers, researchers and care providers across East Toronto, our plan will help our partner organizations better support people in East Toronto during COVID-19 recovery, especially those who have been disproportionately impacted by the pandemic.
We look forward to sharing progress on our cQIP as we continue our work with partners to reach our goals. See our cQIP for 2022/23 (Download PDF) and learn more about our cQIP work.
