
East Toronto HART Hub
The East Toronto Homeless and Addiction Recovery Treatment (HART) Hub provides evidence-based healthcare, treatments, social services and housing support to individuals who actively use drugs or who intend to stop.
We offer low-barrier access to primary care, mental health support and treatment, addiction counselling, direct referral to housing and other support services.
The East Toronto HART Hub has two locations:
- A Danforth site (1156 Danforth Ave., second floor), that focuses on providing primary healthcare services and short-term case management.
- A Coxwell site (688 Coxwell Ave.), that focuses on providing housing support and long-term case management.
The HART Hub is an initiative of the East Toronto Health Partners (ETHP) Ontario Health Team (OHT) and is led by South Riverdale Community Health Centre and St. Michael's Homes, with support from Anishnawbe Health Toronto and additional community partners.

Danforth site: 1156 Danforth Ave., second floor (Greenwood/Danforth)
Monday to Friday, 9:30 a.m. to 4:30 p.m.
Coxwell site: 688 Coxwell Ave., first floor (Coxwell/Danforth)
Monday to Saturday, 9:30 a.m. to 4:30 p.m.
Our services
- Short- and long-term case management
- Day programs, including social recreation groups and therapeutic groups
- Access to primary care, substance use care, and psychiatry
- Counselling and psychotherapy
- Housing support
- Help with basic needs in a safe space
- Information and referrals to other hub services

Walk-ins:
Clients can walk into either HART Hub location during regular hours on a drop-in basis to access service.
The Danforth site (1156 Danforth Ave., second floor) is focused on primary care and short-term case management, while the Coxwell site (688 Coxwell Ave.) is focused on housing support. However, service users can access all services when connecting with either location.
