A-Way is no ordinary courier company. It was founded in 1987 by people with lived experiences of mental health issues to provide meaningful work for all survivors. We provide opportunities for employment, structure and income, as well as community and support to address isolation.
Access Alliance provides services and addresses system inequities to improve health outcomes for the most vulnerable immigrants, refugees and their communities. We provide accessible, community-governed, inter-professional primary health care services, including health promotion, illness prevention and treatment, chronic disease management, and individual and community capacity building. We also offer a range of community wellness programing.
The Albany Medical Clinic has been operating for 75 years and continues to provide comprehensive primary care services to our community. Our clinic offers diagnostic imaging, walk-in clinic, lab, pharmacy, and specialist services, all under one roof. Our team serves over 50,000 patients from the Broadview-Danforth and Mid -Toronto community, as well as the larger East Toronto community.
Alpha House is an addiction recovery community addressing the needs of self-identified men and non-binary people ages 18 and up. We believe recovery from addiction and mental illnesses must address every aspect of the individual. Our programs address physical, mental, spiritual and social needs, all in an effort to support the individual’s recovery from addiction.
Alternatives is a community-based mental health agency that provides one-to-one client case management, supportive counselling and community supports to individuals who have significant mental health challenges. Operated by the Society of Saint Vincent de Paul, Amélie House is a long-term transitional housing facility for women who have experienced difficulty in maintaining adequate permanent housing. We provide safe accommodation, nutritious meals, informal counselling, crisis intervention, referrals, advocacy and basic necessities to the women in our care.
Aurora House provides shelter, long-term trauma counselling, basic needs and essential support services to those who have been trafficked or otherwise exploited in Canada. Seniors Services and Long-Term Care is responsible for service planning and strategic integration of City services for seniors. Services provided include community support programs such as adult day programs, supportive housing services, tenancy supports and homemakers and nurses services for vulnerable individuals who reside in the community and 10 directly operated long-term care homes.

Community Living Toronto has been a source of support for people with an intellectual disability and their families since 1948. We offer a wide range of services including respite, person-directed planning, employment supports, residential programs and community-based activities. Comprehensive Treatment Clinic is a new type of addictions clinic that offers various treatment options for substance use, including methadone, suboxone, sublocade, probuphine, naltrexone and stimulant replacement. We also offer our patients counselling and therapy, safer using supplies, free meals and a weekly art group.
Cota is an accredited, not-for-profit, community-based organization which supports adults with mental health and cognitive challenges to live well within their communities. We provide person-centred supports that assist individuals to find inspiration and hope in their inherent strengths and support them to pursue meaningful change in their lives, as they define it.
Dixon Hall is a multi-service agency serving around 10,000 people annually, including the most vulnerable and the most at-risk members of our community. We offer more than 60 programs and services, and focus primarily on housing and homelessness, seniors’ services, employment services and supports for young people through general programming and music education.
East End Community Health Centre provides primary care, health promotion and disease prevention services to the southeastern quadrant of central Toronto. We focus on the whole person in our approach and recognize that factors such as income, education, relationships and housing play a key role in physical and mental health.

East York Meals on Wheels is a not-for-profit community service agency that delivers great tasting, balanced, nutritious meals through the help of dedicated volunteers. Elizabeth Fry Toronto works with women and non-binary people to create meaningful change in their lives and help them avoid conflict with the law. We offer gender-based, trauma-informed services, advocate for alternatives to the criminal justice system and raise awareness about systemic oppressions that put women and non-binary people in conflict with the law.
Entité 3 is one of six French Language Health Planning Entities mandated by the Ministry of Health to help Francophones access French language health services. We engage the Francophone community to identify their health needs and priorities and help care providers fulfill their French language service obligations by providing strategies, tools and access to training.
Flemingdon Health Centre provides free comprehensive health services, programs and support to improve the well-being of everyone in our communities. Gerstein Crisis Centre provides 24-hour community-based crisis response, as well as a range of intervention services, wellness and recovery groups, and community training for people living with mental health and substance use issues in Toronto. We offer an environment and support that are individualized, responsive and respectful of every client’s autonomy and dignity.
Good Shepherd Non Profit Homes provides permanent supportive housing, congregate living group homes and community-based programs to individuals living with mental health- and substance use-related needs. We help people with a history of or who are at risk of homelessness so they can maintain permanent housing and increase their quality of life.
Good Shepherd Ministries operates a meal program, clothing room, medical clinic and 97 shelter beds for homeless men. We offer support for homeless men seeking abstinence-based treatment, help with finding housing and services for veterans. We also operate two supportive residences, one for seniors with a history of homelessness and one for people living with AIDS.
The GTA Rehab Network is a recognized regional leader in improving the planning and delivery of rehabilitative care. We bring a system-level perspective to issues impacting rehabilitative care, supporting collaboration among our members and partners to create the most effective, coordinated system of rehabilitative care for the GTA.

Health Access Thorncliffe Park supports your physical health and emotional and social well-being, and connects you to any services or supports you may need. Home Instead offers medical and non-medical support for seniors, wherever they call home. Homes First provides homes, shelter and support services to over 1,500 seniors, families and single adults every single night. We own and/or manage 24 properties across the GTA. Michael Garron Hospital (MGH), a division of Toronto East Health Network, is a vibrant community teaching hospital serving one of Canada’s most diverse communities. Nisbet Lodge is a not-for-profit, faith-based long-term care home and seniors housing provider that has served the residents and families of East Toronto for over 45 years. Oakdale Child and Family Service provides long- and short-term care in residential program facilities registered under the Child, Youth and Family Services Act. St. Michael’s Homes provides a continuous-care model of programs and services, including 75 beds in four residential settings and a drop-in service for clients experiencing mental health issues living in the community. We provide pre-treatment, residential treatment, transitional housing and aftercare to men looking to begin their recovery from problematic substance use.
Stella’s Place provides free mental health services for young adults ages 16 to 29. We have a holistic menu of programs, including peer support, clinical services, employment support, wellness, art and recovery programs. We work to help young adults learn skills, build community and be empowered to manage their own mental health.
Street Haven is a women’s organization that provides a variety of services at multiple sites across Toronto. Our services are inclusive, trauma-informed and delivered through a gender-based practice. They include emergency shelters, support housing, in-resident addiction treatment, training and pre-employment supports.
Strides Toronto is a multi-service agency addressing the needs of children, youth and families with mental health challenges. Toronto Paramedic Services is responsible for all aspects of land ambulance service for the City of Toronto. Our mission is to protect and improve the quality of life in Toronto by providing superior and compassionate pre-hospital and out-of-hospital, paramedic-based health care.
Trafalgar offers virtual intensive outpatient and residential addiction and mental health treatment.
