
Primary and Community Care (PCC) Response Teams
The Primary and Community Care (PCC) Response Teams are an integrated care planning program offered by East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto. We offer neighbourhood- and team-based health and social services care planning to help vulnerable adults navigate the healthcare system. This helps adults who may not have a family doctor, may be housebound and/or have unmet health or social needs by connecting them with additional supports and services that are close to home.
Our Response Teams involve many different healthcare and social services professionals representing more than 20 organizations in East Toronto. These include family physicians, home care coordinators, nurse practitioners, case managers, mental health practitioners, supportive housing workers, volunteer community health ambassadors and others. By working together within their local communities, our Response Teams create integrated, comprehensive coordinated care plans for vulnerable adults that help them meet their health goals at home and in their community.
