ETHP submits 2021/22 year-end report to Ontario Ministry of Health

East Toronto Health Partners, the Ontario Health Team (OHT) serving East Toronto, has submitted its year-end report for 2021/22 to the Ontario Ministry of Health.

Highlights of the report include our ongoing collaborative COVID-19 response, with includes our equity-driven strategy for delivering COVID-19 vaccines to East Toronto residents and others. Over the past year, this has involved vaccinating children and, more recently, providing fourth doses to eligible seniors and other vulnerable populations.

The report also highlights our partnership’s work in setting up N95 mask fit testing for frontline staff at community agencies, primary care practices and our other member organizations. In addition, the report notes the launch of our new website and service directory, where you can search more than 1,000 health, social and community services in East Toronto.

Additional highlights include updates on Coached Corridor, an initiative led by East Toronto Family Practice Network (EasT-FPN) that aims to expand access to primary care, including family doctors, in the Gerrard Street corridor; our Community Advisory Council’s 2021/22 Annual Report; our Collaborative Quality Improvement Plan for 2022/23; and a number of other initiatives designed to advance integrated health and social care in East Toronto.
