East Toronto Health Partners Ontario Health Team (ETHP OHT) is proud to launch its new strategic plan.
The plan includes a commitment to our shared purpose: Building a healthier and more equitable East Toronto – enabling every person and neighbourhood to thrive.
Our strategy is to continue working together with our communities to improve the health and well-being of our population over the next 10+ years. In the next five years, we will build a strong foundation in primary care to support a healthier and more equitable East Toronto.
Aligned with this strategy, ETHP OHT and East Toronto Family Practice Network (EasT-FPN) are committed to working with our partners and community to:
- Support access to team-based primary care for 100% of residents in East Toronto
- Ensure every community in East Toronto will have an integrated network of primary care, health and social care partners working together to deliver care
- Improve the health and well-being of our population by working with our communities and partners to redesign care, reduce health inequities and improve health outcomes for the people of East Toronto
Plan shaped by community consultations and strategy sessions
During summer 2024, ETHP OHT launched its strategic planning process to develop a shared set of priorities for our partners and communities. Over the past year, through extensive consultations with community members and advisors, partner organizations, committees and workgroups, as well as with Ontario Health and the Ministry of Health, we identified a set of strategic goals and priorities to guide us for the next several years.
In our most recent East Toronto planning session in June 2025, we had the opportunity to collectively review the draft strategy and discuss how we will balance our efforts to deliver on both provincial goals for OHTs and local priorities for our East Toronto community and partners. Over the summer, we finalized the strategic plan based on feedback we heard.
We were humbled by the many people who contributed their insights, ideas and hopes for the future of integrated care in East Toronto.
