East Toronto Health Partners (ETHP) is inviting all patients, caregivers and community members in East Toronto to complete an anonymous survey on digital equity. The survey aims to capture information about individuals’ knowledge of and access to digital devices in the delivery of care. Your feedback will help us design better health, social and community care programs and ensure they are as equitable and accessible as possible for everyone.

The survey is a joint initiative of ETHP, the Ontario Health Team serving East Toronto, and North Western Toronto Ontario Health Team. Ontario Health Teams are groups of healthcare providers – including hospitals, doctors, home and community care providers and more – who work together as one coordinated team to provide care for you.
ETHP and North Western Toronto Ontario Health Team will review the survey results and conduct focus groups in February 2024 to learn more about digital equity in the communities they serve.
