ETHP Collaborate
ETHP Collaborate is an online collaboration platform that is being piloted to support East Toronto Care Network (EastTCaN), an integrated care planning program offered by East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto.
ETHP Collaborate facilitates integrated care planning supported by East Toronto Care Network (EastTCaN) through:
- Providing a centralized place for providers to refer clients and submit de-identified cases;
- Enabling the creation of and access to live, amendable Coordinated Care Plans (CCPs) for clients;
- Facilitating secure communication among the care and support team;
- Supporting virtual care planning rounds; and
- Documenting the care and support provided by the health and social service providers with clients living in East Toronto.
Note: ETHP Collaborate does NOT replace existing tools such as electronic medical records (EMR) and ConnectingOntario, which may already be used by providers to document care for clients. ETHP Collaborate complements these tools.
Users are responsible for ensuring information related to Coordinated Care Plans (CCPs) on ETHP Collaborate is kept accurate and up to date.
New users of ETHP Collaborate
Health and social service providers who work with East Toronto Care Network (EastTCaN) must register for ETHP Collaborate to refer clients, submit de-identified cases, and access your clients' Coordinated Care Plans (CCPs). These providers must currently be participating in active care planning for clients in the health and social service system.
After you register, you will receive an email confirmation that your registration has been submitted. Please allow up to three business days for your request to be approved. For support, please contact our team at ETHPAdminSupport@tehn.ca.
Existing users of ETHP Collaborate
- We encourage you to bookmark this page on your Internet browser for easy sign-in access to ETHP Collaborate.
- Before you refer a client to East Toronto Care Network (EastTCaN) for rounding support, you must obtain informed consent from the client.
- Download and use our consent form to obtain informed consent from your client before you refer them.
- See instructions for how to refer a client who has provided informed consent.
- If the client does not provide informed consent, you can submit a de-identified case for discussion at rounds.
- Please see the ETHP Collaborate Toolkit (see below) for tools and documents that will help you use the platform effectively.

ETHP Collaborate Toolkit
Users who have successfully registered for ETHP Collaborate have access to this toolkit through the button below. These users will also receive a link to this toolkit by email.
The toolkit includes include videos, workflows, instructions, privacy policies and other materials that help health and social service providers use ETHP Collaborate effectively. ETHP Collaborate users must review all materials in the toolkit before referring clients, submitting de-identified cases, and accessing their clients’ Coordinated Care Plans (CCPs).
The toolkit is hosted on Microsoft SharePoint. Once you have successfully registered for ETHP Collaborate, please bookmark the toolkit for easy access. If you are experiencing technical difficulties, please try opening the link in an Incognito window by right-clicking the button below and selecting the Incognito window option. Future of ETHP Collaborate
ETHP Collaborate was developed by East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto, to empower community care teams. By leveraging Microsoft's robust technology, ETHP Collaborate enhances healthcare collaboration, enabling community care teams to deliver comprehensive and coordinated patient care. This supports improved outcomes and enhanced client experiences, while prioritizing security and privacy standards.
As part of the first phase of its rollout across the OHT, ETHP Collaborate will support East Toronto Care Network (EastTCaN) operations. Please stay tuned for updates related to subsequent phases of the rollout.
