United in knowledge: ETHP participates in North American Conference on Integrated Care 2024

East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto, recently celebrated a significant milestone with its participation in this year’s North American Conference on Integrated Care (NACIC) from October 15 to 17. 

Hosted in Calgary, Alberta, the global event brought together healthcare leaders to explore transformative approaches in integrated care. Partners from diverse ETHP member organizations conducted 14 presentations that showcased initiatives spanning citizen research, equity data collection, community-based network building, and innovative models in chronic disease management and preventative care. 

Each of these presentations reflected our OHT’s dedication to co-designing integrated care solutions with community members, aligning our healthcare strategies with the needs of the neighbourhoods we serve. This further highlighted our commitment to building a healthier and more equitable East Toronto – enabling every person and neighbourhood to thrive.

Participating partners included individuals from East Toronto Family Practice Network, Flemingdon Health Centre, Michael Garron Hospital, Spectrum Health Care, TNO – The Neighborhood Organization, VHA Home HealthCare and WoodGreen Community Services.  

Key insights and reflections

NACIC emphasized the nine pillars of integrated care, which are championed by the International Foundation for Integrated Care (IFIC) and aligned with ETHP’s approach to addressing health inequities and improving accessibility. 

The event reinforced several essential themes that will shape our OHT’s path forward: 

  • The power of cross-sector partnerships: Working across organizations enables us to drive system-wide change more effectively. 
  • Innovative equity and accessibility initiatives: New integrated care models are essential for sustainable reform. 
  • Evidence-based practices for impact: Embedding evidence in practice was a common thread, reinforcing our commitment to effective, data-informed solutions. 

Outside of the conference’s formal presentations, informal conversations with peers enriched our team’s understanding of best practices, inspiring new ideas to bring back to our communities. 

Moving forward together

Our participation at NACIC 2024 was more than just a success – it was a collective experience that emphasized the power of teamwork and cross-disciplinary collaboration. 

As we apply the knowledge and insights gained across our OHT, our shared vision remains clear: a more integrated, equitable healthcare system. 

We look forward to continuing our work with renewed energy and sharing these experiences within our networks to create lasting, positive change.
