East Toronto Health Partners (ETHP), the Ontario Health Team (OHT) serving East Toronto, has submitted its year-end report for 2022/23 to the Ontario Ministry of Health.
Highlights of the report include our ongoing efforts to improve access to health, social and community services to more than 300,000 East Toronto residents through a number of initiatives. These include:
- Launching online services and preparing for in-person services at the Thorncliffe Park Youth Wellness Hub, a safe, welcoming space for youth to get services related to health, wellness, employment, housing and more.
- Opening Health Access Taylor-Massey, a healthcare, community and social services centre that offers connections to services and resources to people who live in the Taylor-Massey neighbourhood and surrounding areas.
- Launching a series of free educational videos to help unpaid caregivers find the support they need for their care recipients and themselves.
- Improving home and community-based care for older adults, including through our home care modernization leading project, which was approved to enter the next steps, and the launch of MGH2Home, an enhanced care program that helps patients transition back to home safely when they leave Michael Garron Hospital.
The report also highlights our continued work with patient, caregiver and community advisors; community health ambassadors; and primary care providers, including family doctors and nurse practitioners, across East Toronto.
In addition, the report outlines our OHT’s collaborative efforts to develop tools and resources that help people more easily find health, social and community services near them.
